Let’s start where most clinics won’t. BBL has historically carried the highest complication rate of any cosmetic procedure, and that reputation is deserved — it’s why some surgeons in the UK stopped offering it altogether for a period. But the risk was never really about the procedure itself. It was about where the fat is injected, and that is now well understood.
Fat injected too deep — into or beneath the gluteal muscle — can enter a large vein and travel to the lungs. That’s a fat embolism, and it’s the complication behind almost every serious BBL case you may have read about. Injected into the subcutaneous layer, above the muscle, that pathway doesn’t exist. This is the single most important thing to understand about BBL safety, and the single most important question to ask any surgeon: “Which plane do you inject into?”
If a clinic can’t answer that clearly, or brushes it off, you have your answer.
A €1,200 BBL isn’t a discount — it’s a different procedure. At that price something has been removed from the equation: the hospital becomes an outpatient room, the anaesthetist becomes a nurse, the surgeon may not be a certified plastic surgeon at all, and the time spent injecting carefully becomes time saved. The implant is placed intramuscularly — inside the gluteal muscle — which is what keeps it stable, well covered and natural in appearance. Before you consider adding volume, it’s worth checking whether volume is actually what’s missing. If your buttocks have dropped rather than deflated — if the skin has lost its tone after weight loss, pregnancy or simply with age, and the crease underneath has softened or lengthened — then adding fat or an implant won’t fix it. It will add volume to tissue that’s already sitting too low, and in some cases make the sagging more obvious. There’s a simple check you can do right now, in front of a mirror. Lift the tissue with your hands and see what happens. If lifting it gives you the shape you want, the problem is position — you need a lift. If it still looks flat when lifted, the problem is volume — you need fat transfer or an implant. This is the requirement people underestimate. A BBL doesn’t create volume, it moves it, so there has to be a donor site — usually the abdomen, waist, lower back or thighs. Very slim patients often can’t provide enough for a meaningful result, and a surgeon who promises otherwise is either planning to under-deliver or to over-harvest, which is its own risk. Your surgeon works with the body you have on the day. Lose fifteen kilos afterwards and you lose part of the transferred fat along with it; gain fifteen and the untreated areas change around your new shape. Two patients with identical fat reserves can need entirely different procedures, and skin is the difference. Elastic skin holds transferred fat in a smooth, rounded shape.
