Why one-sided enlargement needs assessment
Gynecomastia can affect the two sides differently. However, a new unilateral lump or change should not be assumed to be benign gland enlargement from photographs alone. Tell the clinician when it appeared, whether it is growing and whether there is pain, discharge or a skin change.
The NHS gynecomastia guidance supports medical assessment of breast symptoms. Investigation, when indicated, comes before a cosmetic operation. A reassuring remote conversation cannot replace examination of a concerning lump.
How the technique is chosen
The surgeon assesses fat, gland, skin and nipple position on both sides. The smaller side provides a useful comparison, but it is not necessarily a perfectly symmetrical target: chest-wall shape, muscle and natural nipple position can differ too.

Will the other side need treatment?
Not necessarily. Ask the surgeon to show which differences come from excess tissue and which come from underlying anatomy. Treating an otherwise acceptable side adds an intervention and should have a clear reason, not be presented as automatically necessary for symmetry.
Agree what “balanced” means in your case. Discuss nipple height, areola appearance, chest contour and the likely scar. Perfect mirror-image symmetry is not a realistic guarantee, and temporary differences can be more obvious during healing.
Why the price is not simply halved
Facility preparation, anaesthesia staffing, assessment and recovery care still have costs when only one side is treated. The amount of operating time and complexity may also differ from what the external size suggests.

Recovery and symmetry review
The untreated side will not go through the same surgical swelling, so early comparison can be misleading. Follow the instructions for activity, dressings and any compression garment. Do not use extra pressure on the operated side to force a faster match.
ASPS describes a settling period of several months for final results. Ask when formal photographs and contour assessment are planned. Persistent differences may warrant review, but the timing of any revision should be individualised.
Symptoms that need attention
