Calling an operation “under sedation” does not fully describe its pain-control plan. Ask whether local anaesthetic will also be used and what depth of sedation is intended. Sedation exists on a spectrum, so it should not automatically be described as complete sleep or guaranteed lack of memory.

Do not make your own fasting decision from the table. Ask the facility exactly when to stop food and liquids and which medicines to take. Plans can change if another procedure is added, so ensure the instructions describe the current operation.
When local anaesthesia alone may be discussed
For a suitable limited correction, remaining awake may be acceptable to the patient and surgeon. You can usually communicate sensations and ask questions during treatment. A previous comfortable experience with local anaesthetic may help you explain your preferences, but it does not establish suitability for this operation.

When sedation may be discussed
Significant anxiety, difficulty tolerating an awake procedure or a more involved treatment plan may lead to a sedation discussion. It also introduces additional assessment, monitoring and recovery requirements. Tell the clinician about sleep apnoea, breathing difficulties, alcohol use, sedating medicines and previous anaesthetic problems.
Sedation should be delivered in an appropriate setting by a team equipped to recognise and manage problems. Ask who will monitor you and how an unexpected change in breathing or responsiveness would be handled. A lower price should not decide the staffing or monitoring arrangements.
Questions that settle the decision
Ask the team to explain why the recommended option is suitable for your procedure. Clarify whether the quote includes the anaesthesia clinician and observation period. Find out what would happen if local anaesthesia proved insufficient or you became too uncomfortable to continue.
